Bring your driver’s license and insurance cards with you. Co-payment for insurance is due at time of service. We accept cash, check, Visa, MasterCard or Discover.
Wear loose, comfortable clothing that is big enough to accommodate a large bandage after surgery. Wear comfortable shoes such as slip-on, no high heels.
Do not wear any jewelry (including body piercing), makeup or cologne.
Do NOT wear contact lenses. We provide containers for removable dentures and bridgework.
Sleep apnea patients please bring your CPAP machine.
If your child is having surgery, please bring a favorite stuffed animal or security blanket and a sippy cup.
